By Vinay Mishra Delhi: Safdarjung Hospital Surgeons Successfully Remove 10.1 kg Retroperitoneal Tumour from 19-Year-Old GirlThe Surgery OPD at Safdarjung Hospital recently received a 19-year-old girl with progressive abdominal distension over the past five years. Despite seeking care in multiple hospitals, her family was still awaiting treatment and losing hope. She was admitted immediately, investigated, and prepared for exploratory laparotomy within a week of presentation.Pre-operative imaging revealed a massive retroperitoneal tumour measuring approximately 30 x 20 x 20 cm arising near the crus of the left diaphragm and occupying the entire upper abdomen.
The tumour was abutting major vessels, displacing vital organs, stretching the renal hilum, and pushing the left kidney down to the suprapubic region with abnormal rotation. Its composition suggested a likely mature teratoma.During surgery, the team discovered the tumour filling the whole abdomen and pushing abdominal viscera to the right flank. En-mass excision was extremely challenging due to its size, weight, and proximity to vital structures. The tumour had feeders from multiple major vessels, including the left adrenal and splenic vessels, which were carefully preserved. The spleen, kidney, stomach, duodenum, pancreas, intestines, and colon were all repositioned to their normal anatomical locations after removal. The left crus of diaphragm and left pleura were repaired, and a chest drain was placed.
The excised tumour weighed an extraordinary 10.1 kg and was removed uneventfully.This remarkable surgical success was possible under the dynamic leadership and guidance of Director Dr. Sandeep Bansal, Principal Dr. Geetika Khanna, Medical Superintendent Dr. Charu Bamba, and HOD Anaesthesia Dr. Kavita Sharma. The dedicated OT staff, ICU team, and HDU ward staff, along with nursing and physiotherapy services, played a critical role in the patient's smooth recovery.After initial ICU care and continued monitoring in HDU ward 24, the patient made a complete recovery and was discharged.
